    The film opens with the sudden death of Erica Schulman, the CEO of Atlanta Memorial Hospital, who was admitted for a minor heart irregularity. Medical Examiner Jonas McLellan, who is dating Hailey Dean, discovers that Erica was poisoned with a massive overdose of digoxin, a drug derived from the poisonous foxglove plant. Detective Monty initially focuses on Dr. Meghan Phillips, Hailey’s close friend, due to a public argument the two women had regarding patient care. However, the mystery deepens when a second victim, Sarah Van Valkenburg, dies under identical circumstances. Hailey begins profiling the hospital staff, looking for an 'Angel of Death'—a killer who believes they are ending patient suffering. Suspects include the arrogant Dr. Tim Chang, who was denied a prestigious golf club membership by the victims, and Anne the pharmacist, who feared Erica’s automation plans would cost her her job. Parallel to the mystery, Hailey prepares to meet Jonas’s father, Grant, a retired accountant struggling to find a new purpose. This subplot provides comedic relief as Grant attempts to master 'Quiche à la Grant' (a disastrous dish involving maple syrup) before eventually finding his true calling in woodworking. Meanwhile, District Attorney Paulina faces a professional crisis when Clayton Morrel, a murderer she convicted years ago, is granted parole despite her best efforts to keep him incarcerated. Back at the hospital, a third victim, Mary Smith, is discovered. The investigation shifts toward Brian Martin, a nurse with a history of 'mercy killing' at a previous hospital. Hailey, however, is skeptical of the 'Angel of Death' theory because the murders are too organized and the victims weren't terminal. While investigating the hospital late at night, Hailey is attacked and drugged with morphine. She survives and eventually connects the dots: Ivy, Erica’s seemingly timid assistant, was in a secret relationship with Robert Harms, the man who became interim CEO after Erica’s death. Ivy killed Erica to clear the path for Robert’s promotion and to eliminate her romantic rival. She then killed the other patients to frame Brian as a serial killer. In the climax, after Robert breaks up with Ivy, she attempts to kill him and Hailey with digoxin, but Fincher and the police arrive just in time to apprehend her. The film ends with a cliffhanger as the paroled Clayton Morrel ominously confronts Paulina, signaling future danger.
